Visa Waiver for child under 14

I think I have to sign the green visa waiver for my daughter who is 11. I know on the front I put in her details but on the back under the questions which I answer no to , do I put my name, date of birth and then sign for it. Can't remember what I did last time so if anyone knows it would be helpful.

You put all yoru daughters details on the form, and just sign it yourself.

My son has signed his own the last 2 years and he is only 10 years old

I don't think they worry who signs it, but suggest you fill it in so there are no mistakes

I fill in and my son signs his(12)
