I know this has been discussed several times but I'd be grateful if someone who point me to a link where I can find samples of the form that need to be completed before entry into the States.

When we came out to Vegas there was a party of school children on and I can't tell you how many trees they must have used filling them in!!
You would think they would have sat them down in the airport and done it, instead they walked up and down the plane and kept giving them new ones, even the steward ended up checking them!!
