Visa waiver

I have looked at the post containing the sample visa waiver form and see that a parent needs to sign for a child under 14. In the last part of the form above my signature would I put my name or my childs name. Not sure if they are asking for the person whose signature it is date of birth or the childs at the bottom.

Sorry if a stupid question just a bit confused and want to do it right.

I always put the child's name and just signed with mine. Never had a problem doing this.
