Employment history

One of the visa forms needs employment history. How far do I need to go back in time? Can't remember all the dates and theres four places for different employment details. I can fill 3 up, roughly guessing the third ones dates but do I need to put four places?

Appreciate finding out how much you filled in. It doesn't say like some forms need 10 year work history.

We just put hubbys last three jobs on his form (2 of them since gone out of business so don't know how relevant they were!)
