what is a supplemental non immigrant visa? and who needs one ?

If you are applying for a visa then all males between the age of 16 years ( might be 14?) and 50 years need to fill in the supplemental info form. You can find it on line along with visa application forms.

That's right. It is called DS-157 and my Bother-in-law had to fill it out.
