My husband had to apply for a visa, as he had a conviction. He was granted a 10 year one. My question is - does he have to fill in the visa waiver form on the plane? I would have thought not as he has got a visa, but somebody told me that the airline staff say that everybody on the plane has to fill one in.

When they bring round the green visa waiver forms they also bring around a similar white form which you must complete if you are travelling on a visa. You do not need to fill in a green visa waiver.
You need to ask for the visa holders form. Liz is right, it is white. On most charter etc flights, thry will assume you are on visa waiver unless you point out differently.
